Registration Instructions

1. Submit Registration Form
Please complete the registration form and fax the completed form to 800-498-5719.

2. Submit Payment
Once the Registration Form is received, we will issue an electronic invoice that you will receive via email. Just follow the easy steps to pay online or follow the steps in the Payment Info section of the registration form.

3. Submit the Team Roster Form
To ensure that we have a valid count of total participants, please submit the team roster form.

4. Submit the Participant Waiver Form for Each Participant
For insurance purposes, we must have the Participant Waiver Form on file for each participant. If this form is not submitted, the athlete will not be allowed to participate.
